Responsibilities:

Provide direct patient care including administering medications, performing wound care, and assisting with ADLs Accurately document patient information, progress, and concerns in a timely and thorough manner Develop and implement individualized care plans to meet the unique needs of each resident Collaborate with interdisciplinary teams to ensure seamless communication and coordination of care Maintain a clean and safe environment, adhering to infection control and safety protocols Provide emotional support and comfort to residents and families during challenging times Licensed Practical Nurse (LPN)
