Job Summary:

Rosewood Rehabilitation & Nursing Center is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Weekend Baylor Program team! As an LPN, you will play a vital role in providing high-quality patient care to our residents, under the supervision of our experienced healthcare team. Our Weekend Baylor Program is a unique and rewarding opportunity to work with our residents on weekends, with flexible scheduling options. Licensed Practical Nurse (LPN)

Responsibilities:

Provide direct patient care, including administering medications Monitor and report changes in patient condition to the registered nurse or physician Collaborate with the interdisciplinary team to develop and implement individualized patient care plans Maintain accurate and detailed records of patient care and treatments Participate in quality improvement initiatives to optimize patient outcomes and satisfaction Foster a positive and respectful environment that promotes patient-centered care Licensed Practical Nurse (LPN)

Requirements:

Current LPN licensure in the state of Pennsylvania High school diploma or equivalent required; some college or nursing program education preferred Prior experience in a healthcare setting; previous experience in long-term care or rehabilitation setting a plus Strong communication and interpersonal skills; ability to work effectively with patients, families, and healthcare team members
