Provide high-quality, patient-centered care to our residents, focusing on their unique needs and goals Collaborate with the interdisciplinary team to develop and implement individualized care plans Administer medications, perform treatments, and provide vital signs as needed Monitor resident progress, report changes, and make adjustments to care plans as necessary Engage with residents and their families to build strong relationships and promote a sense of community Participate in educational opportunities and training programs to stay current with industry best practices

Requirements:

Current LPN licensure in the state of

Pennsylvania Work Schedule:

Weekend shifts, including Saturdays and Sundays and One shift during the week.
