We are actively filling full-time , part-time , and per diem positions across all shifts (days, evenings, nights). Join us immediately to make a direct impact! Licensed Practical Nurses (LPNs) Key Responsibilities Deliver direct nursing care per physician orders and personalized resident care plans Administer medications and treatments safely and precisely Monitor resident health closely; report any changes promptly to RNs or physicians Document all care and services accurately in the electronic medical record Support seamless admissions, discharges, transfers, and care transitions Supervise and direct CNAs in daily resident support activities Maintain rigorous compliance with infection control, safety protocols, and regulatory requirements Foster effective, compassionate communication with residents, families, and the healthcare team Licensed Practical Nurses (LPNs) Qualifications Active Licensed Practical Nurse (LPN) license in Pennsylvania (required) Graduate of an accredited practical nursing program Experience in long-term care, rehabilitation, or skilled nursing preferred (new graduates and returning professionals are encouraged to apply!)
