Chaim Gitelis DO PC is seeking a motivated and dependable Licensed Practical Nurse (LPN) to join our growing Remote Patient Monitoring (RPM) Program as a Field Support LPN . This is a field-based leadership position supporting multiple partner skilled nursing and rehabilitation facilities. The Field Support LPN serves as an extension of the Field Manager, ensuring the successful operation of the RPM program while providing clinical support, staff training, and operational oversight. Why Join Our Team? ✅ Full-Time Salaried Position ($50,000/year) ✅ Monday-Friday Schedule ✅ Leadership & Career Growth Opportunities ✅ Paid Training ✅ Monthly Bonus Opportunities ✅ Work Directly with Providers and Clinical Leadership ✅ Dynamic Role with Variety Every Day What You'll Do Clinical Support Collect and document vital signs for residents enrolled in the RPM program Identify and communicate abnormal vital signs to the clinical team Facilitate telehealth visits using company-provided technology Complete required documentation accurately and timely Field Leadership Serve as an extension of the Field Manager across assigned facilities Train, onboard, and support new CNA Vitals Specialists Provide ongoing coaching and guidance to field staff Troubleshoot equipment, tablets, and operational issues Assist with inventory management and equipment deployment Support quality assurance initiatives and maintain program standards Build positive relationships with facility staff and leadership Escalate operational concerns and assist with problem resolution Qualifications Active LPN license Minimum one (1) year of nursing experience preferred Leadership, supervisory, or training experience is a plus Strong communication and organizational skills Comfortable using tablets and healthcare technology Ability to work independently while supporting multiple facilities Reliable transportation required Must be willing and able to travel between assigned partner facilities throughout the workday
