Job description Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to inform care plans. Administer medications and injections accurately following prescribed protocols, including IV infusions and medication administration. Assist with complex procedures such as catheterization, tube feeding, wound suturing, and sterile processing techniques. Manage patient documentation meticulously using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems like Epic, Cerner, Athenahealth, or eClinicalWorks. Provide specialized care for patients with conditions such as Alzheimer's disease, dementia, traumatic injuries (Level I & II trauma centers), infectious diseases, and post-acute recovery needs. Support emergency response efforts by managing airway management, ventilator use, and acute pain management in fast-paced environments like ICU or emergency room settings. Collaborate with interdisciplinary teams on discharge planning, case management, and health coaching to ensure comprehensive patient care across inpatient and outpatient services. Requirements Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice. Proven experience in critical care environments such as ICU or trauma centers is highly desirable. Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, or Athenahealth; familiarity with

ICD-9/10

coding essential for accurate medical documentation. Experience working with diverse patient populations including pediatrics, geriatrics, individuals with developmental disabilities or memory care needs. Knowledge of infection control practices, aseptic techniques, and HIPAA regulations to ensure patient safety and confidentiality. Ability to perform venipuncture (venipuncture), phlebotomy, catheter care, and other technical skills required for comprehensive nursing care. Strong communication skills for triage assessments and collaborating within multidisciplinary teams in fast-paced settings such as emergency medicine or urgent care.
