Job Description

Delta Health is looking for a Licensed Practical Nurse (LPN) / Patient Access Specialist to join our Contact Center team — the heartbeat of our patient communication and care coordination. This is a unique hybrid role where you'll make a difference from the very first patient call to the final follow-up. What You'll Do In this dual clinical and administrative role, you will: Clinical & Patient Support Serve as a key clinical contact for patients by phone — sharing test results, medication updates, and provider instructions.
Provide education, reassurance, and follow-up support within your scope of practice.
Assist with routine clinical procedures, medication administration, wound care, specimen collection, and prescription refills.
Document accurately in the EHR and collaborate closely with providers and nursing staff.
Float to clinic locations as needed to support patient care. Patient Access Responsibilities Register patients, verify insurance, and obtain necessary authorizations.
Schedule, reschedule, and confirm appointments while ensuring smooth patient flow.
Collect co-pays/deductibles and assist patients with financial counseling referrals.
Manage clerical tasks and maintain accurate documentation in the EHR.Coordinate care between patients, providers, and departments to ensure timely communication. What You Bring High School Diploma or equivalentGraduate of an accredited LPN programCurrent Colorado LPN license (or ability to obtain within 60 days)BLS certification requiredIV certification (upon hire or within 1 year)2+ years of experience in a clinic or patient access setting (call center experience preferred)Strong communication skills with a calm, clear, and empathetic phone presenceProficiency with medical terminology, insurance verification, EHR systems, and basic mathOrganized, adaptable, and able to work independently in a fast-paced environmentBilingual (English/Spanish) preferred Why Join Delta Health?
