LVN Intake Specialist Full-Time | Remote - Must live near one of these locations - Lubbock, TX / Norman, OK / Paris, TX / McKinney, TX Were looking for a detail-driven LVN Intake Specialist to join our team. In this role, youll manage incoming referrals, ensure documentation is accurate and compliant, and help coordinate a smooth admission process for patients. This is a great opportunity for someone who thrives in a fast-paced environment and wants to play a key role in patient care coordination. What Youll Do Process and enter referrals in HCHB accurately and in a timely mannerReview and approve Face-to-Face documentation to ensure Medicare complianceEnsure all referral documentation is complete; coordinate with DONs as neededRun daily reports and resolve any missing or incorrect informationPartner with Admission Coordinators to move pending referrals to admissionCommunicate with physician offices and internal teams to gather required infoMaintain continuity by ensuring coverage and handoff of pending referralsSupport the Intake team and assist with additional tasks as needed What Were Looking For Active LVN/LPN license (compact or multi-state) in good standing2 years of Home Health experience strongly preferredMust live in Norman, OK, Paris, TX, McKinney, TX, or Lubbock, TXExperience in home health intake or similar healthcare setting preferredFamiliarity with HCHB and/or EMR systems is a plusKnowledge of medical terminology and ICD-10 preferredStrong attention to detail and accuracyComfortable communicating with physicians and clinical teamsHighly organized with the ability to multitask in a fast-paced environmentStrong computer skills (typing, systems navigation, documentation)Team-oriented with a positive, flexible attitude Why Join Us Fully remote position with a supportive teamOpportunity to play a critical role in patient care coordinationStable, full-time scheduleSimilar jobs in McKinney, TX
