WeCare Centers at Foltsbrook Care Center in Herkimer, NY is seeking an experienced LPNAC to join our team on a full-time, hybrid, basis. In this role, you'll support our nursing facility by completing MDS assessments and ensuring accurate documentation for quality resident care and proper reimbursement.
