English Attention to detail Patient interaction Care coordination Home & community care experience Full Job Description About the Role We are seeking an experienced Licensed Vocational Nurse (LVN) to support our Remote Patient Monitoring (RPM) program. This fully remote role involves monitoring patients' daily biometric readings, providing telephonic outreach, and supporting chronic care management. Candidate must reside in Southern California. Bilingual Spanish fluency is required to effectively assist our diverse patient population. Responsibilities Review patient biometric data (BP, weight, glucose, pulse ox, etc.) through the RPM platform Respond to alerts and escalations per clinical protocols Conduct patient outreach calls to assess symptoms and provide education Communicate clearly and effectively with Spanish-speaking patients Document all patient interactions and interventions accurately in the system Coordinate care with RNs, physicians, and care team members Assist patients with RPM device setup and troubleshooting Maintain compliance with
HIPAA, CMS RPM
guidelines, and internal policies Qualifications Active LVN license (California preferred)
Bilingual:
Fluent in English and Spanish (required) 1-2 years clinical experience (telehealth, home health, primary care, or chronic disease management preferred) Strong communication and customer-service skills Comfortable working with digital health tools and remote technology Detail-oriented and able to work independently
