Job Description

Responsibilities/Job Description:
Fairview is looking to hire a new LPN to join out Transplant team. This is a full time (80 hours per pay period) benefit eligible position. We believe the hours for this position will be Monday-Friday 8a-5p; this is a mobile position, meaning you will be expected to be onsite 2 days a week and will be able to work from home the other days. As a member of the interdependent team led by a designated RN, the LPN is accountable to perform patient care duties and activities for a group of patients. Responsibilities Maintains knowledge of, and complies with, all relevant laws, regulations, policies, procedures and standards. Actively participates in creating and implementing improvements. Supports the nursing process to achieve outcome focused patient care by following the plan of care in order to achieve identified patient outcomes. Observes and collects data to contribute to the assessments of the patient and family in order to develop an outcome focused plan of care. Follows the plan of care in order to achieve identified patient outcomes. Intervenes in emergency situations as appropriate within scope of practice. Communicates effectively with patient/family, physicians, and the interdependent team regarding the care of the patient. Contributes to the achievement of patient and unit outcomes (Safety, cost, quality, satisfaction, engagement) by working collaboratively with inter and intradepartmental teams. Utilizes the nursing process to provide holistic, compassionate, safe, high quality, population-based care Observes and collects pertinent data and/or clinical findings to enhance patient outcomes. Provides patient educational information appropriate to the developmental level, learning needs, readiness, cultural values, and beliefs of patients and families. Demonstrates practices that support safety with recognition of variations in populations served, preventing harm to patients, families, employees, and themselves. Demonstrates behavior consistent with the standards, scope of practice, ethics, and characteristics of a licensed professional. Basic ability to use a computer and applications that are associated with performing basic work tasks (navigate in Windows, Outlook, epic EHR, etc.). Skilled in developing effective rapport with customers, co- workers, or families, actively listening to develop a positive connection. Ability to interact effectively, listen actively and develop positive working relationships with diverse customers and staff honoring different world views. Required Qualifications Practical Nursing diploma from an accredited school of nursing or Trade/Vocational Certification 1-3 years experience Drivers License Some positions may require a valid driver's license and insurance for home visits Basic Life Support (American Heart Assoc or Red Cross) MN Licensed Practical Nurse (LPN)
Qualifications:
$26.58- $37.53 Hourly
