Location:
Remote- Must reside in New Hampshire with occasional local travel to hospitals and skilled nursing facilities throughout the state
Employment Type:
Full-Time Salary:
$80,000- 95,000 annually, based on licensure and experience Position Overview A leading managed care organization serving New Hampshire Medicaid members is seeking an experienced and detail-oriented nurse to support long-term care and rehabilitation admissions throughout the state.
- Coordinate admissions into long-term care, skilled nursing facilities, and rehabilitation programs across New Hampshire
- Review clinical documentation to determine appropriate level of care and placement eligibility
- Facilitate transitions from hospitals to SNFs or long-term care settings
- Serve as liaison between the health plan, facility admissions teams, and hospital discharge planners
- Manage prior authorizations and eligibility verification within scope of licensure
- Collaborate with utilization management, case management, and care coordination teams
- Educate members and families regarding Medicaid benefits and care transitions
- Maintain accurate documentation in EMR and care coordination systems
- Ensure compliance with New Hampshire Medicaid guidelines and long-term care regulations Qualifications
- Active, unrestricted New Hampshire RN or LPN license required
- Minimum 2-3 years of experience in long-term care, home health, rehabilitation, or hospital admissions
- Working knowledge of New Hampshire Medicaid long-term care eligibility and level-of-care criteria
- Strong communication, organization, and documentation skills
- Ability to work independently in a remote environment
- Proficiency with electronic medical records and care coordination platforms Preferred Background
- Managed care or health plan experience
- Utilization management or case management experience
- Experience with hospital discharge planning or SNF placement
- Experience working with New Hampshire Medicaid waiver programs or long-term services and supports (LTSS)
- Bilingual English and Spanish a plus Why Join
