Job Description

If so, consider joining the Somerset Nursing & Rehabilitation Center team! We currently have a dynamic career opportunity available for a qualified MDS Coordinator. This is a part-time remote hybrid position.
MDS Coordinator Requirements:
Applicant must possess strong communication, clinical, and organizational skills. MDS experience preferred. Current/active RN or LPN licensure required.
MDS Coordinator Job duties:
Transcribe and take appropriate action on physician's orders as directed including but not limited to Care Plan changes. Assist with performing all admitting and discharge assessments and complete all other necessary paperwork. Help monitor all aspects of patient care. Initiate requests for consultation or referral. Respond to requests from the resident, physician, or nursing staff. Cooperate with other resident services when coordinating nursing services to ensure that the resident's total regimen of care is maintained. Coordinate and prepare MDS assessments for all residents. Oversee the transmission of MDS resident assessments. Prepare, monitor, and update assessments, Care Area Assessments, and Care Plans, as necessary. Participate in Care Plan meetings with residents and family members. Be aware of residents' exit to and reentry from hospitals.
