Job Summary We are seeking an experienced and detail-oriented, rockstar MDS Coordinator to join our team! In this role, you will be the clinical detective of our documentation. Your primary mission is to dig deep into patient records to ensure our assessments accurately capture the full scope of high-quality care our nursing team provides. This precision directly ensures appropriate reimbursement and compliance for our facility. •Prior MDS experience is
REQUIRED.
• Key Responsibilities Complete Minimum Data Set (MDS) assessments accurately and timely. Audit clinical documentation to find missed care details. Coordinate the care planning process with facility IDT. Monitor Quality Measures to identify areas for improvement. Submit assessment data to state and federal databases. No ON-CALL rotation! Qualifications Active nursing license (RN or LPN/LVN) in Colorado. Minimum 2 years of experience managing the MDS process. Deep knowledge of PDPM and Resident Assessment Instrument (RAI) guidelines. Exceptional analytical skills with a passion for thorough documentation. Strong work ethic and a collaborative, team-first attitude.
