Flexible scheduling:

Per Diem | PRN | Part-Time | Contract 1:1 patient-focused care in a home setting Supportive clinical leadership Meaningful, relationship-driven work Opportunity to practice with autonomy and strong clinical judgment What You'll Do as an LPN Conduct re-assessments and supervisory visits Implement individualized plans of care Administer and document oral, topical, subcutaneous, and IM medications Monitor for and report adverse reactions to RN supervision Provide skilled wound care and initiate emergency measures when needed Maintain accurate, timely clinical documentation Perform all procedures within LPN scope and agency protocols Ensure professional, respectful communication with clients and families Uphold HIPAA standards and strict client confidentiality What We're Looking For Graduate of a National League for Nursing-accredited program Active LPN license Minimum 1 year nursing experience (home care preferred) Experience with bowel & bladder care strongly preferred Current CPR certification Valid driver's license & automobile insurance Negative TB test (or TB blood test) Who Thrives Here?
