Part-Time - 2 days/week 12 hour shifts 7am-7pm; weekend shift differential •Sign-on Bonus if orientation completed by 7/31/26 Job Overview We are seeking a dedicated and compassionate Nurse to join our healthcare team. The ideal candidate will possess a strong commitment to patient care and have the ability to work in a fast-paced environment. As a Staff Nurse, you will be responsible for providing high-quality nursing care to patients, collaborating with healthcare professionals, and ensuring the delivery of effective treatment plans. This is for residents in a long-term care community as well as patients in short-term rehab therapy, enjoy the opportunity to connect with patients and their families. Responsibilities Conduct thorough patient assessments and develop individualized care plans. Administer medications and treatments as prescribed by physicians. Monitor patients' vital signs and report any changes in condition to the healthcare team. Provide education and support to patients and their families regarding health management and treatment options. Assist in case management, ensuring continuity of care for patients with complex health needs. Collaborate with interdisciplinary teams to coordinate care for patients, including those requiring infectious disease care or ostomy care. Utilize electronic health record systems such as Point Click Care for documentation and patient management. Skills Proficient in patient assessment techniques and clinical decision-making. Knowledge of coding practices relevant to nursing documentation. Familiarity with case management principles and practices. Experience in infectious disease care is preferred. Strong communication skills, both verbal and written, for effective patient interaction. Ability to work collaboratively within a multidisciplinary team environment.
