Job Description

Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic clinic team. In this vital role, you will provide high-quality patient care by performing comprehensive assessments, administering treatments, and supporting patients across a variety of healthcare settings. Your expertise will help ensure smooth clinic operations, promote patient well-being, and uphold the highest standards of safety and confidentiality. This position offers an exciting opportunity to make a meaningful difference in patients' lives while working in a fast-paced, collaborative environment. Responsibilities Conduct thorough patient assessments, including vital signs, medical history intake, and physical examinations using your physiology knowledge. Administer medications and injections accurately following prescribed protocols, including IV infusions and medication administration. Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures. Support patients through procedures like catheterization, tube feeding, wound suturing, and sterile techniques ensuring infection control standards are maintained. Manage patient documentation efficiently within EMR (Electronic Medical Records) systems such as Epic or Athenahealth, ensuring HIPAA compliance. Provide patient education on health coaching topics including medication management, nutrition, dementia care, Alzheimer's care, and chronic disease management. Collaborate with multidisciplinary teams in various settings such as urgent care, outpatient clinics, long-term care facilities, hospice care, or hospital environments including ICU or trauma centers. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN). Proven experience working with diverse patient populations in a clinic settings. Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding, and documentation standards. Experience with EMR/EHR systems. eClinicalWorks is highly preferred. Demonstrated ability to perform venipuncture, phlebotomy, injection administration, and IV insertion with sterile technique. Familiarity with infection control practices and aseptic techniques across various clinical procedures. Excellent communication skills for triage assessments and patient education. Ability to work effectively with individuals with disabilities or developmental delays in settings like assisted living or home & community care environments. $25.00 - $27.00 per hour
