QUALIFICATIONS
: Current S.C. Licensure as L.P.N. or R.N, and a valid South Carolina driver's license with a good driving record.SKILLS REQUIRED
Ability to communicate effectively, both in written and oral forms; knowledge of training programs, ability to work effectively with staff, people we support and their families. Must be able to perform physical and visual skills as listed: balancing, stooping, kneeling, crouching, reaching, standing, walking, pushing, lifting, grasping, talking, hearing and visual acuity. Successful completion of orientation program.JOB SUMMARY
: Provides individualized support or training to people in the community residences according to local, state and federal guidelines. Implements individualized training goals, collects data and related paperwork. Assist and support people as necessary. Provide individualized SERVICE and support to eligible individuals as defined by theSC Department of Disabilities and Special Needs :

JOB DUTIES
Is familiar with and protects resident's legal rights and reports violations or suspected violations of these rights. As an employee by law you must report immediately violations or suspected violations of abuse, neglect, exploitation to the appropriate authority i.e.REPORT ALL ABUSE TO SLED
1-866-200-6066 or State Long Term Care Ombudsman Program 1-800-868-9095. Be familiar with and works to protect peoples' legal rights. a. Provides and assists direct support staff in the provision of the medical care which meets individual's needs. Services are to be rendered in accordance with federal, state Medicaid/Medicare and DHEC guidelines. b. Develops, revise, update and participates in the developments or implementation of individual's program plans. Participates as a member of the Interdisciplinary Team. c. Develops medical care plans which physician has determined that an individual required such a plan. a. Review health status of individuals by physical exam as required or as needed. Refers to physician for health problems. b. Provides any nursing care as prescribed by the physician. c. Trains staff in detecting signs/symptoms of illnesses or dysfunction, first aid procedures, and basic non-professional skills needed to meet health needs of the individuals (s). Perform supervised medical passes upon completion of Med-Tech training program for staff successfully completing the course requirements. d. Responsible for seeing that all individuals are escorted to medical appointments. e. Transcribes physician orders, administers routine oral, subcutaneous, intramuscular medications following applicable guidelines and procedures. f. Charts significant responses to medications and documents treatments or illnesses. g. Charts at a minimal weekly progress of each individual. Charts as necessary changes of an individual. h. Responsible for TB Control for the staff and residents of all CTH II and CRCF programs. i. Administer Heptavax Series to staff as needed j. Responsible for overseeing that Utilization Reviews and obtaining Level of Care Information, k. Assist Residential Director in maintain Infection Control Policy for the residential Programs. l. Responsible for assuring implementation of Pharmacy meetings at least quarterly. m. Train as needed individuals on health care and sexual awareness goals. n. Assures completion of nursing, medical, pharmacy, self-administration and sexual awareness assessments. o. Performs other related duties as required/ On-call for residential direct support professionals/individual emergencies weekend will be rotated with on call nurse. p. Attend the following agency meetings as scheduled but not limited to: annual individual program plans, Human Rights Committee, Safety meeting etc.WORK HOURS
generally 8:00 am until 4:00 pm (subject to change depending on job duties). It is expected this position will work different shifts to ensure cross training.Benefits:
