Our team is committed to improving patient outcomes and enhancing quality of life through innovative treatment approaches. Overview We are seeking a dedicated and detail-oriented Chronic Care Management (CCM) Care Coordinator who holds credentials such as CMA (Certified Medical Assistant), RMA (Registered Medical Assistant), or LPN (Licensed Practical Nurse). This role serves as a primary point of contact for patients with chronic conditions, providing consistent outreach, education, and care coordination primarily through secure texting and phone communication. The Care Coordinator builds trusted relationships with patients, monitors health updates, identifies clinical concerns, and ensures timely communication with providers — helping patients stay supported and safely managed between office visits. This position requires strong written communication, sound clinical judgment, excellent organization, and comfort working across multiple healthcare systems while maintaining accurate, Medicare-compliant documentation.
Key Responsibilities:
Communicate with CCM patients via secure messaging and phone Build rapport and provide patient-centered education and support Review charts, care plans, and medications prior to responding Identify and escalate clinical concerns appropriately Document all interactions accurately in the EHR and CCM platforms Support Medicare CCM documentation and compliance
Qualifications:
Excellent written communication and attention to detail Strong organization and ability to manage multiple patient conversations Comfort with EHRs, messaging platforms, and healthcare systems Ability to manage high volume while maintaining accuracy and personalization
We Offer:
This position is 5 days a week 8am-4:30pm Mon•Thur 8am•Noon on Friday Office located in North Charleston Salary based on the candidate's experience and licensure
Pay:
$17.00•$21.00 per hour
