Job Description

Essential Duties and Responsibilities Prepare patients for examinations and treatments by obtaining and documenting vital signs, medical histories, chief complaints, and medication information. Assist providers during examinations, procedures, and treatments. Administer medications, immunizations, injections, and treatments as prescribed and within the scope of practice. Perform point-of-care testing, specimen collection, and basic laboratory procedures. Document patient information accurately and timely in the electronic health record (EHR). Coordinate patient care activities, including referrals, follow-up appointments, and patient education. Respond to patient inquiries and triage patient concerns according to established protocols. Assist with chronic disease management and preventive care initiatives. Maintain exam rooms, clinical supplies, and equipment to ensure readiness and compliance with infection control standards. Participate in quality improvement, risk management, and patient safety activities. Ensure compliance with FQHC, state, federal, OSHA, HIPAA, and HRSA requirements. Support care coordination efforts and collaborate with interdisciplinary healthcare team members. Participate in outreach and population health initiatives as needed. Perform other duties as assigned. Education Graduate of an accredited Practical Nursing program. Licensure and Certification Current Licensed Practical Nurse (LPN) license in the State of South Carolina. Current Basic Life Support (BLS) certification required. Experience Minimum one (1) year of nursing experience preferred. Experience in primary care, family practice, community health, or an FQHC setting preferred. Experience working with diverse and underserved populations preferred. The Licensed Practical Nurse (LPN) provides direct patient care and clinical support services under the supervision of a physician, nurse practitioner, physician assistant, or registered nurse. The LPN plays a vital role in delivering high-quality, patient-centered care within a Federally Qualified Health Center (FQHC) environment, supporting the organization's mission to provide accessible and comprehensive healthcare services to underserved populations.
