Job Description

Careteam Plus serves the Grand Strand areas including Myrtle Beach, Georgetown, Williamsburg, Pawleys Island, and Conway, SC. We are a primary and specialty care healthcare organization providing comprehensive services for patients of all ages—from newborns to seniors. Position:
Licensed Practical Nurse (LPN) The Licensed Practical Nurse (LPN) delivers patient care within the scope of practice defined by South Carolina state law and the policies and procedures of Careteam Plus. The LPN supports patient health by gathering health data, assisting with assessments, administering medications and treatments, conducting provider-ordered diagnostic tests, and ensuring thorough documentation. This role also involves patient education, medication reconciliation, and care coordination as part of a collaborative clinical team.
QUALIFICATIONS
Experience Minimum of one year of direct patient care experience in a primary care setting required. Experience using Athena EMR preferred. Certification, License, Registration Current and active LPN license in South Carolina (SCLLR) required (or eligibility for licensure). Current Basic Life Support (BLS) certification required. Special Skills Bilingual fluency in English and Spanish preferred. Strong customer service and communication skills required. Ability to multitask, stay organized, and adapt to shifting priorities. Demonstrated sound clinical judgment and effective problem-solving skills. Ability to complete delegated tasks with accountability. Proficiency in PC and EMR systems; Microsoft Office suite proficiency required. Solid math skills including basic arithmetic and understanding of ratios and percentages. Other Must meet Careteam Plus employee health requirements for Hepatitis B and Tuberculosis screenings.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Conduct patient interviews to verify information and medical history.
Prepare patients for exams:
perform vital signs checks, collect preliminary health data. Assist providers in coordinating referrals and follow-up care per established protocols. Conduct occupational and health screening tests as ordered. Perform CLIA-waived lab tests and document quality control procedures. Accurately record all elements of care: notes, treatments, immunizations, medications, discharge instructions, and follow-up arrangements. Partner with providers to assess patient education needs and deliver relevant instruction. Respond appropriately to emergency situations and deliver care as directed. Administer prescribed medications and treatments. Maintain accurate and up-to-date electronic medical records (EHR). Ensure a safe, clean, and infection-controlled clinical environment. Safeguard patient privacy and handle protected health information in compliance with HIPAA. Assist with patient inquiries and follow-up visits. Support pre-authorization and pre-certification processes as needed.
Monitor and coordinate care:
follow up on no-shows, pending labs, imaging, referrals, and return visits. Help with patient rescheduling as necessary. Conduct pre-visit planning in accordance with clinical protocols. Participate in Quality Improvement (QI) initiatives and relevant meetings. Utilize EHR effectively for quality control, data validation, coordination with external providers, and accurate documentation. Collaborate with team members to support overall clinic operations and patient needs. Perform other duties as assigned. Careteam Plus is an Equal Opportunity Employer.
