Job Description

Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise will help ensure patients receive personalized, efficient, and safe nursing services. Duties Administer medications and injections accurately following physician orders, including IV infusions and other specialized treatments Perform comprehensive patient assessments, vital signs monitoring, and physical examinations to evaluate health status Assist with medical procedures such as catheterization, suturing, and specimen collection while maintaining sterile technique Document patient care activities thoroughly using electronic health record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations Provide wound care management, including dressing changes and infection control measures for diverse patient populations Support discharge planning and coordinate with case management teams to ensure seamless transitions of care Educate patients and their families on health management strategies, medication administration, nutrition, and disease prevention Experience Valid licensure as a Licensed Practical Nurse in accordance with state regulations Prior experience working in critical care environments such as ICU or Level I Trauma Centers is highly desirable Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth for accurate medical documentation Experience providing specialized care including Alzheimer's/dementia/memory care, hospice, pediatric or neonatal care preferred Knowledge of
ICD-9/ICD-10
coding for accurate medical record keeping and billing processes Background in diverse settings like long-term care facilities, hospitals, emergency rooms, or outpatient clinics is advantageous Strong understanding of physiology, anatomy, pharmacology principles, and infection control practices Join us to deliver exceptional patient-centered care that transforms lives!
