Job Description

Job Overview We are seeking a dedicated and compassionate Long Term Care Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong background in patient care within long-term care settings, demonstrating expertise in managing complex health needs, providing personalized nursing care, and supporting residents' quality of life. The successful applicant will be committed to delivering high standards of care while adhering to all regulatory guidelines and safety protocols. Duties Provide comprehensive nursing care to residents in long-term care facilities, including medication administration, wound management, and vital signs monitoring. Assist with activities of daily living such as bathing, dressing, mobility support, and nutritional intake to promote independence and well-being. Conduct thorough patient assessments, document medical histories, and update electronic health records (EHR) systems accurately and promptly. Collaborate with interdisciplinary teams to develop individualized care plans tailored to each resident's needs, including dementia care and Alzheimer's support. Administer IV infusions, manage catheterizations, and perform sterile procedures following aseptic techniques. Monitor for signs of acute pain or infection, providing timely interventions and triage as necessary. Support discharge planning and coordinate with family members or caregivers to ensure continuity of care upon transition or transfer. Follow HIPAA regulations diligently to maintain patient confidentiality and ensure compliance with healthcare standards. Experience Valid LPN license with current state certification required; additional certifications in IV therapy or hospice care are advantageous. Prior experience working in long-term care facilities such as nursing homes or assisted living communities is essential. Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate documentation. Experience in critical care environments such as ICU or Level I Trauma Centers enhances clinical skills applicable to complex resident needs. Knowledge of ICD coding (ICD-9/ICD-10), medication administration protocols, and infection control practices is highly desirable. Background working with individuals with developmental disabilities or behavioral health conditions is preferred. Familiarity with medical imaging procedures, wound care techniques, and pain management strategies will support comprehensive resident care.
