LPN (Licensed Practical Nurse) FT Monday through Thursday 2p-10p

Monday through Thursday Second Shift LPN 2P-10P and every other Friday 2p-10p

Responsibilities:

• Provide direct patient care, including administering medications, performing treatments, and monitoring vital signs
• Collaborate with healthcare team members to develop and implement patient care plans
• Assist with patient assessments and document findings accurately
• Educate patients and their families on healthcare procedures, medications, and self-care techniques
• Maintain accurate and up-to-date patient records
• Adhere to infection control protocols and maintain a clean and safe environment for patients
• Respond to emergencies and provide appropriate interventions

Experience:

• Minimum of 1 year of experience as a Licensed Practical Nurse (LPN)
• Minimum of 1 year of experience with a medication cart
• Experience in pain management, dementia care, hospice care, and health coaching preferred
• Proficiency in using electronic medical record systems such as Eldermark
• Strong medication administration skills

Qualifications:

• Valid LPN license in the state of Tennessee
• CPR certification
• Excellent communication and interpersonal skills
• Ability to work effectively in a fast-paced healthcare environment
• Strong attention to detail and ability to multitask
