Decatur Wellness & Rehab is seeking compassionate, reliable LPNs to join our care team. If you're looking for a rewarding role in a team-driven environment, we want to meet you! LPN Position Summary As a LPN at Decatur Wellness & Rehab, you'll be an essential part of our residents' care.

Responsibilities include:

Direct the daily activities of the CNAs and ensure policies and procedures are followed by nursing personnel. Participate in admissions, discharges and transfers of residents as required. Assess resident's conditions, prepare, and administer medications per physician orders. Initiate, review, and update care plans. Manage the nursing care for all residents and delegate shift tasks to nurses. Supervise nurse aides as they complete their assignments. Observe residents' conditions, oversee, and respond as necessary. Inform physicians of any changes in resident conditions. Maintain confidentiality of protected health information, including verbal, written and electronic communications.
