Job Description
BRIEF JOBSUMMARY
The LPN Trainer is responsible for direct support of the Individual Served in the home and during community activities. The goal of the LPN Trainer is to ensure that all assigned Individual Served receive the necessary care to achieve a safe, meaningful, and positive home experience. It is the responsibility of the LPN Trainer to develop and teach Individual Served skills that will enhance their level of independence in the home and community setting. The goal for all Individual Served is to achieve total independence in their home and community environments.ESSENTIAL JOB FUNCTIONS
: Train and teach independence skills in the home environment to each assigned Person Served based on the goals and training objectives stated in the Individual Support Plan, this includes the implementation of all therapeutic plans. Maintain cleanliness inside and outside the home and report needed repairs to the Residential Home Coordinator as identified to ensure a safe home environment. Teach and train Person Served in home maintenance skills. Document daily Person Served activities, along with their response to the activities in the Person Served Daily Logs; this must be completed at the end of each shift. Prepare and serve meals specific to Person Served specified diets as stated in the Individual Support Plan or Therapeutic Plans. Assist Person Served in completing daily personal hygiene tasks to the extent required by the Person Served. Promote and encourage the Person Served to learn to be independent in completing these tasks to the maximum extent possible. Ensure that all medications, both prescription and over the counter, are administered and documented accurately on the Medication Administration Record. Attend all related in-service training and residential meetings as required by the agency in a timely manner and understand that obtaining re-training before expiration is your required duty. Training is the responsibility of the staff and knowing the expiration of all training is staff's responsibility as well. Conduct employment in a professional and positive manner and communicate professionally with Person Served and their family members, ISC's, therapists, and any other agency stakeholder. Speaking to each visitor to the home in a direct polite manner to greet them appropriately. Transport and accompany Individual Served on community outings of their choice to promote positive activities for the Individual Served; teach and train Individual Served skills that help to achieve independence in the community. Provide medical care to the Individual Served according to the specific Nursing Plan of Care. Ensure all ordered medications and treatments are provided to the Individual Served according to the Plan of Care and document actions in the Nursing Progress Notes.
