Substance Abuse & Behavioral Health Treatment About the Role PH Wellness is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to support clients through substance abuse detoxification and residential treatment. This role performs comprehensive assessments, oversees medication self‑administration, and ensures a safe, supportive environment in accordance with LPN scope of practice and DHS Incidental Medical Services (IMS) standards. Key Responsibilities Complete nursing assessments at admission, including medical history and substance use details. Monitor clients throughout detox, documenting vital signs, withdrawal symptoms, and health changes each shift. Oversee client self‑administration of medications and monitor for adverse reactions or emergencies. Collaborate with physicians, HCP, and treatment team to implement individualized detox and treatment plans. Provide client education on detox expectations, medication effects, and withdrawal management. Conduct ongoing health assessments and report concerns to the treatment team and HCP. Respond to medical emergencies and coordinate with emergency services when needed. Support integrated care for clients with co‑occurring mental health conditions. Maintain accurate Medication Observation Records and electronic medical documentation. Uphold PH Wellness policies, including HIPAA and medication management guidelines. Assist in training and supporting behavioral health technicians and medical team members. Qualifications Active LPN license in good standing CPR/First Aid certification Required continuing education completed Experience in detox, behavioral health, or residential treatment preferred Strong communication, assessment, and documentation skills Ability to maintain professionalism, empathy, and boundaries in a treatment setting Additional Requirements Ability to work within LPN scope and seek HCP guidance when needed Strong teamwork and collaboration skills Reliable, organized, and able to respond promptly to client needs Why Join PH Wellness? $25.00 - $28.00 per hour
