Job Description

Overview Join our dynamic healthcare team as a Licensed Practical Nurse (LPN), where your expertise will directly impact patient care and well-being. In this vital role, you will provide compassionate, efficient nursing services across various settings, including long-term care facilities, hospitals, outpatient clinics, and home health environments. Your skills in patient assessment, medication administration, and medical documentation will help ensure high-quality care delivery. Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately following prescribed protocols, including IV infusions and sterile procedures. Assist with diagnostic evaluations such as specimen collection, laboratory processing, and medical imaging preparations. Manage airway management techniques, catheterization procedures, tube feeding, and ventilator support in accordance with best practices. Provide specialized care for patients with complex needs such as dementia, Alzheimer's disease, memory care, or those requiring hospice or palliative care services. Collaborate with interdisciplinary teams to develop discharge plans, case management strategies, and coordinate post-acute or home & community care. Maintain detailed medical records ensuring compliance with HIPAA regulations and accurate coding using ICD-9/10 and CPT standards for billing purposes. Support emergency response efforts in urgent or trauma situations by applying Level I/II trauma center protocols or ICU procedures as needed. Educate patients and families on health coaching topics including medication management, nutrition, pain control, and disease prevention strategies. Experience Valid state licensure as a Licensed Practical Nurse with current certification. Proven experience working in diverse clinical settings such as hospitals (including emergency room, ICU, PICU), nursing homes, assisted living facilities, or outpatient clinics. Critical care experience preferred; familiarity with Level I/II trauma centers or acute care environments highly valued. Hands-on experience with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks for documentation and patient management. Strong knowledge of medical terminology, physiology, anatomy, infection control practices, and aseptic techniques. Experience administering IV infusions and injections; competent in venipuncture and catheter care procedures. Background working with individuals with disabilities or special needs including dementia or developmental disabilities is advantageous. Ability to perform physical examinations and assessments accurately while demonstrating excellent patient care skills in fast-paced settings such as urgent care or hospital medicine environments.
