Job Description

MDS Coordinator - Come join our team at Magnolia Ridge! Our skilled nursing facility has an immediate opening for MDS Coordinator. The ideal candidate will be a licensed in the State of Tennessee, with a love of servant leadership. Alongside the clinical management team, the MDS Coordinator will execute assessments and interventions as each resident needs to reduce poor outcomes and increase health and well-being! We are in the heart of Union City, TN! Apply today and become the next member of our team! Required Qualifications Must possess a current, active license to practice as a
RN / LPN
in this state. 2 years' experience as a RN / LPN, one of which includes supervisory experience, preferred. Experience with MDS completion preferred. Resident Assessment Coordinator (RAC) Certification is preferred. Understanding of computer technology, including Electronic Health Record (HER) systems Major Duties and Responsibilities Coordination of the facility's comprehensive resident assessment process in accordance with state and federal regulations. Accurate completion of all MDS assessments and any supporting assessments or clinical documentation. Implementation and ongoing evaluation of each resident's comprehensive plan of care. Scheduling and leading interdisciplinary care plan for team meetings. Reviewing medical records for the presence of supporting documentation for all items coded on the MDS. Provide education to department heads, physicians, and other staff as needed. Collaborating with health plans, physicians and the interdisciplinary team for assuring insurance approvals and/or that residents meet eligibility and coverage criteria. Interpreting rules, regulations and coverage guidelines and acting as primary resource for problem solving regarding care-based reimbursement systems and quality reporting programs. Interact and collaborate with third-party and managed care insurance systems, providing the necessary resident centered care-based documentation necessary for evaluation and reimbursement.
