You will:

Conduct resident assessments and develop individualized care plans Manage and administer medications and treatments safely and accurately Monitor and evaluate resident conditions and adjust care as needed Ensure compliance with state regulations and clinical standards Provide delegation and oversight to caregiving staff Complete required documentation and charting Support residents with dignity, compassion, and respect Participate in on-call rotation or ensure appropriate clinical coverage Key Responsibilities Complete on-site assessments at least every 45 days Provide clinical oversight and task delegation to team members Maintain accurate, timely documentation Collaborate with interdisciplinary team members to enhance care outcomes Support both Memory Care and Assisted Living populations What We're Looking For ✔ Active LPN license in Tennessee ✔ Experience in assisted living, long-term care, or memory care preferred ✔ Strong clinical judgment and organizational skills ✔ Passion for working with seniors ✔ Team-oriented mindset with leadership capability ✔ Ability to thrive in a fast-paced, resident-centered environment Our Core Values in Action At Charter, we don't just talk about our values—we live them every day: Serve with Heart
