  • 2026, and we are hiring Licensed Practical Nurses (LPNs) to join our specialized Transition Team.
About the Transition Team Our Transition Team is a supplemental care unit of highly skilled nurses whose primary focus is to quickly and efficiently support patients transitioning home while permanent nursing staff are actively recruited. This team is deployed immediately after a patient referral is received and the patient is admitted to Maxim. Transition Team nurses provide nursing care for all authorized hours in the home during a provisional period of 1-90 days. During this time, our office team recruits permanent caregivers. Once identified, those caregivers are given the opportunity to train and shadow alongside Transition Team nurses in the home before assuming independent care. After the transition is complete, Transition Team nurses move on to support the next new admission—providing variety, consistency, and meaningful impact in patient care. Why Join Maxim?
