8-Hour Day Shifts 40 Hours per Week 13-Week Assignment On-call rotation every 3-4 weeks Key Responsibilities Provide skilled nursing care in patients' homes. Assist RN Case Managers with patient assessments and care plans. Document patient care and clinical outcomes accurately. Educate patients and families on self-care techniques. Follow infection control and safety protocols. Coordinate care with interdisciplinary healthcare teams. Requirements Active LPN/LVN License. Minimum 2 years of clinical experience preferred. At least 1 year of Home Health experience. Experience with OASIS documentation. Experience using Homecare Homebase software. Valid Driver's License and reliable transportation. Strong communication and patient care skills. Education Graduate of an accredited Licensed Practical Nursing Program or School of Vocational Nursing. Certifications Current LPN/LVN License Current CPR Certification Preferred Qualifications Home Health or Hospice experience. Experience with electronic medical records. Strong assessment and documentation skills. Ability to work independently in the field.Pay:
$1,800.00- $1,900.
