This is an excellent opportunity for an experienced Home Health LVN/LPN looking for autonomy, meaningful patient relationships, and the ability to make a direct impact in the community.

Schedule:
Monday-Friday 8:00 AM - 5:00
PM Compensation:
$71,000 - $87,000 annually, depending on experience This position is paid on a per-visit basis and offers additional earning potential for nurses who are able to efficiently manage their schedules and patient visits.
Responsibilities:
Provide direct patient care under the supervision of a Registered Nurse Observe, assess, and document patient conditions and changes Administer medications and treatments as prescribed Educate patients and families on care plans and health management Maintain accurate and timely clinical documentation Communicate effectively with physicians, nurses, and interdisciplinary care teams Assist in coordinating patient services and follow-up care Maintain a safe and supportive patient environment
Qualifications:
Graduate of an accredited LVN/LPN program Current Texas LVN or LPN license in good standing Minimum one year of LVN/LPN experience Current CPR certification Valid driver's license and reliable transportation Ability to travel throughout assigned coverage area Strong clinical assessment and documentation skills
