Full Job Description Licensed Vocational Nurse Job Description Title:
Licensed Vocational Nurse (LVN)

Director of Nursing Job Summary Performs all health histories and clinic intake for medical patients. Responsible for patient appointments, medication, referral to specialists, follow up documentation and any other clinical matters that may occur. Responsibilities Escort patients to exam rooms prepare room/patient for physician exam. Obtain patient medical history, vital signs, weight, and height. Record patients information to include test results into their medical chart. Assists with non-invasive, direct patient care procedures, and assists with medical examinations as specifically directed by provider. Administers injections properly according to CDC and OSHA guidelines as well as internal policies and procedures. Practices appropriate safety, environmental, and infection control procedures. Provides excellent internal/external customer service via telephone, fax or face-to-face contact to assist patients with their health care needs. Assists with scheduling medical referrals and outpatient testing. Understanding your role and responsibilities as a care team member. Participating in care team and staff meetings. Using insight and knowledge to help the patient utilize outreach services when necessary or requested. Using insight and knowledge to help the patient utilize behavioral health services when necessary or requested. Ensuring patients have access to care in an appropriate time frame. Using data and reports to reach out to specific patient populations to ensure patients receive preventive care services, immunizations, and acute or chronic care services we provide. Encourage care managed patients to meet goals and stay on track with care plan that is set by their PCP. Determine if any other outreach services needed for clients. Ensures that the provider flow is maintained appropriately Adhere to HIPAA guidelines. Performs other duties as assigned by supervisor. Minimum Requirements Must have a high school diploma or equivalent and graduated from an accredited school of vocational nursing and current LVN license. 1-3 years experience in a primary health care setting Must be knowledgeable of personal computers to include internet/intranet navigations Strong communication skills Ability to manage confidential records and information Certified in Cardiopulmonary Resuscitation (CPR)
Typical Physical Demands Finger Dexterity:
Using fingers to make small movements such as typing or picking up small objects. Normal fine and gross motor control of fingers and hands.
Talking:
Frequently conveying detailed or important instructions or ideas accurately, clearly, or quickly.
Hearing:
Able to hear average or normal conversations and receive ordinary information; hear whispered voice at five (5) feet.
Repetitive Motions:
Frequently and regularly using the wrists, hands, and fingers.
Visual:
Visual acuity necessary for the proper evaluation of a patient and to prepare or inspect documents or other materials. Ability to accurately view computer monitors
Physical:
Sitting 20-30% of time and standing, walking 70-80% of time. Lift up to 20-50 lbs; push and pull up to 20-50 lbs. Able to bend at the waist, twist and turn along axial plane.
