Field-based / On the road Job Summary We are seeking a compassionate and dependable PRN Licensed Vocational Nurse (LVN) to provide quality nursing care to hospice patients in their place of residence. This role supports patients and families through end-of-life care while working collaboratively with the RN and interdisciplinary team. Key Responsibilities Provide direct patient care in accordance with the established Plan of Care (POC) under the supervision of an RN Monitor and report changes in patient condition to the RN and care team Assist with patient assessments and ongoing care needs Administer medications and treatments as ordered Educate patients and families on basic care, comfort measures, and disease progression Deliver compassionate support to patients and families during end-of-life care Maintain accurate and timely clinical documentation Assist with coordination of care and communication with the interdisciplinary team Participate in on-call coverage as assigned Qualifications Current, active Licensed Vocational Nurse (LVN) license in the state At least 1 year of hospice experience (required) Strong clinical and observational skills Ability to work independently while following RN direction Excellent communication and interpersonal skills Preferred Experience 1-2+ years of LVN experience in a clinical care setting Hospice or home health experience strongly preferred Experience with Consolo EMR preferred

$35.00 - $40.00 per hour
