  • We are seeking a highly motivated and organized MDS Coordinator to join our team at Avir at The Meadow, a premier healthcare provider in Dallas, Texas. As an essential member of our staff, you will play a critical role in ensuring the highest quality of care for our residents.
  • Job Summary:
  • As an MDS Coordinator, you will be responsible for coordinating the Minimum Data Set (MDS) assessment process, ensuring accurate and timely completion of assessments, and maintaining accurate and detailed records.
You will also work closely with the interdisciplinary team to identify and address resident care needs, and provide ongoing support to ensure seamless transitions and optimal resident outcomes.
  • Responsibilities:
  • Coordinate the MDS assessment process, including scheduling assessments, collecting and verifying data, and ensuring timely completion
  • Maintain accurate and detailed records, including MDS assessments, care plans, and service reports
  • Collaborate with the interdisciplinary team to identify and address resident care needs, and develop individualized care plans
  • Provide ongoing support and education to staff, residents, and families, ensuring a positive and inclusive care environment
  • Ensure compliance with regulations, standards, and policies, including Medicare and Medicaid requirements
  • Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance resident care and satisfaction
  • Qualifications:
  • Bachelor's degree in Healthcare Administration, Nursing, or related field
  • (or equivalent experience)
  • 1+ year experience as an MDS Coordinator or in a similar role
  • Strong understanding of MDS assessment process and regulations
  • Excellent communication, organization, and interpersonal skills
  • Ability to work effectively in a fast-paced environment with multiple priorities
