In this vital role, you will provide compassionate, high-quality patient care across various healthcare settings, including inpatient, outpatient, long-term care, and community-based services. Your expertise will support patient assessments, medication administration, wound care, and vital sign monitoring while collaborating with multidisciplinary teams to ensure optimal health outcomes. This position offers flexibility and the opportunity to make a meaningful difference in the lives of diverse patient populations. Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to determine care needs. Administer medications and injections accurately following prescribed protocols, including IV infusions and catheter care. Utilize EMR (Electronic Medical Record) systems such as Epic or Cerner to document patient information meticulously and ensure compliance with HIPAA regulations. Assist with complex procedures such as wound management, tube feeding, venipuncture, and sterile processing techniques. Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments within memory or behavioral health settings. Support acute pain management strategies and participate in discharge planning for post-acute or outpatient care. Collaborate with physicians and other healthcare professionals during diagnostic evaluations, respiratory management (ventilators), and emergency interventions like airway management or trauma response in Level I or Level II trauma centers. Requirements Valid LVN license in good standing with the state licensing board. Proven experience working in critical care environments such as ICU, emergency room, or hospital medicine units. Familiarity with EMR/EHR systems Demonstrated knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/ICD-10), CPT coding, and infection control standards. Experience providing patient care in diverse settings such as assisted living facilities, nursing homes, hospice care, or home & community care environments. Ability to perform phlebotomy, IV insertion/infusion procedures, suturing assistance, and specimen collection with aseptic technique. Strong assessment skills for triage situations and working with individuals with disabilities or developmental delays.
