This is an in office position located in our Dallas, TX office and is a great opportunity to join our growing Nursing team! Job Summar y The LPN/LVN Care Coordinator (LCC) will be responsible for calling in verbal orders on behalf of nursing team members, tracking receipt of signed orders, coordinating daily operations and scheduling needs, and ensuring there is continuity of care. This role will establish and cultivate a positive working relationship with current referrals sources, field clinician, client families, and nursing team members to ensure workflow processes are achieved.
Responsibilities:
Essential Duties and Responsibilities Monitor and maintain monthly & quarterly Key Performance Indicators (KPI's). Call in nursing verbal orders in a timely manner for continuity of care. Assist with contacting doctor offices to verify and confirm receipt of paperwork and status updates on outstanding orders. Verify client primary care physician and well check information as needed. Update and maintain physician's current and accurate information in KanTime system on an ongoing basis. Assist with processing paperwork received daily in KanTime. Maintain open communication with nursing team members, caregivers, and other departments. Assist in order entry and scheduling changes as entered in ticketing system. Comply with all safety policies, practices & procedures & report unsafe activity to your supervisor. Provide leadership to others through example and sharing of knowledge/skills. Participate in proactive team efforts to achieve departmental & company goals. Perform other duties as assigned.
Qualifications:
Other Qualifications and Skills Ability to read & write in simple correspondence with effective public speaking in small groups - may be required in more than one language. Ability to add, subtract, multiply & divide with simple two-digit numbers. Ability to carry out detailed but involved written instructions, able to problem-solve with a few variables. Intermediate computer skills to include Microsoft Office Suite and database activity. Handle multiple tasks simultaneously. Knowledge and experience in the use, delivery methods, dosing, side effects, and drug interactions of medications affecting pediatric populations. Education and Work Experience Requirements Licensure as an LVN/LPN with ability to be credentialed in multiple non-compact states. Two (2) - Five (5) years of nursing experience, pediatrics preferred. Home Health experience is strongly preferred. One (1) - 3 (3) year of administrative experience.
