POSITION SUMMARY/SCOPE
The Licensed Vocational Nurse (LVN) in the Rural Health Clinic provides direct patient care and clinical support services under the supervision of a Registered Nurse (RN), Nurse Practitioner (NP), Physician Assistant (PA), or Physician. The LVN functions within the Texas Board of Nursing scope of practice and supports quality patient care in an outpatient rural setting, ensuring compliance with CMS Rural Health Clinic regulations, clinic policies, and patient safety standards.

Responsibilities:
Clinical Patient Care Perform patient intake, including vital signs, medical history, medication reconciliation, and chief complaint documentation
Assist providers with examinations, procedures, and treatments
Administer medications and injections as ordered and within LVN scope of practice
Perform wound care, dressing changes, suture removal, and basic treatments per provider orders
Collect specimens (urine, swabs, blood via venipuncture if trained/competent) and prepare for lab processing
Perform point-of-care testing (e.g., glucose, urinalysis, rapid strep/flu/COVID testing)
Provide patient education related to medications, treatments, and follow-up care under RN/provider guidance Care Coordination & Documentation Accurately document all patient care, treatments, and observations in the electronic health record (EHR)
Communicate patient condition changes promptly to RN or provider
Assist with referrals, follow-up appointments, and care coordination
Support chronic care management and preventive health services as directed Clinic Operations & Safety Prepare exam rooms and maintain clean, stocked, and organized clinical areas
Follow infection control, OSHA, HIPAA, and safety policies
Assist with vaccine administration clinics and preventive services
Oversee the Texas Vaccines for Children program and Immtrac
Participate in quality improvement initiatives and clinic audits
Support emergency response within clinic scope (e.g., CPR, basic life support) Professional & Team Responsibilities Work collaboratively with providers, RNs, MAs, LVNs, and administrative staff
Participate in staff meetings, training, and competency validation
Maintain required licensure, certifications, and continuing education
Adhere to clinic policies, ethical standards, and professional conduct expectations
Scope of Practice & Supervision:
Practices under the supervision of an RN, NP, PA, or Physician in accordance with Texas Board of Nursing rules
Does not independently assess, diagnose, or prescribe
Escalates clinical concerns appropriately and timely
Physical Requirements:
Ability to stand, walk, and move throughout clinic during shift
Ability to lift and assist patients up to 50 pounds
Manual dexterity for clinical procedures
Visual and auditory acuity for patient assessment and safety

Qualifications Required Qualifications:
Graduate of an accredited Licensed Vocational Nursing program
Current Texas LVN license in good standing
Current BLS (CPR) certification (or obtain within 6 months)
Knowledge of basic clinical procedures and infection control standards
Ability to work independently and as part of a healthcare team in a rural setting
Preferred Qualifications:
Experience in a Rural Health Clinic, outpatient clinic, or primary care setting
Experience with EHR systems
Venipuncture and immunization experience
