Job Description

Position Summary:
We are seeking a dedicated and detail-oriented Licensed Vocational Nurse (LVN) to join our team. The LVN will be responsible for medication management, coordination with healthcare providers, and track accurate documentation to ensure compliance with Texas Health & Human Services Commission Policy. This role requires a high level of organization, communication skills, and accountability.
Key Responsibilities:
Medication Management:
Accurately count and document all medications. Order medication refills as needed to ensure continuous care. Verify that medication labels match MAR (Medication Administration Record) documents. Maintain proper storage and handling of medications according to facility protocols. Coordinate with pharmacies and physicians for new prescriptions, refills, or clarifications. Documentation & Claims submission: Complete and maintain thorough documentation for all nursing activities. Ensure all notes and logs meet billing requirements — a minimum of 40 billable LVN logs/notes per week is expected. Accurately record services provided to ensure compliance with healthcare standards and provider fiscal requirements.
Coordination & Communication:
Communicate effectively with doctors, pharmacists, and interdisciplinary team members (ex. verify physician's orders) Report any medication discrepancies, adverse reactions, or changes in patient condition promptly.
Additional Duties:
Perform other nursing or administrative duties as assigned over time. Participate in ongoing training, quality improvement initiatives, and compliance reviews. Educate & train direct care service personnel for providing quality care to persons requiring assistance.
