Job Overview Join our dynamic healthcare team as a Licensed Vocational Nurse (LVN) working from 2 PM to 10 PM! Duties Perform thorough patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic and Cerner. Administer medications and injections accurately following prescribed protocols, including IV infusions and medication management for complex cases like acute pain and hospice care. Assist with specialized procedures such as catheterization, tube feeding, venipuncture, and sterile dressing changes while maintaining aseptic technique. Monitor patients' conditions closely, utilizing critical care experience to identify changes requiring urgent intervention or triage decisions. Collaborate with interdisciplinary teams to develop and update individualized care plans for patients with diverse needs such as dementia, Alzheimer's disease, or developmental disabilities. Document all patient interactions meticulously in medical records, ensuring compliance with HIPAA regulations and ICD coding standards like ICD-10 and ICD-9. Support discharge planning and post-acute care coordination to ensure seamless transitions for patients returning home or to assisted living facilities. Requirements Valid LVN license in the state of practice with current certification. Proven experience working in various healthcare environments such as hospitals (including Level I Trauma Centers), nursing homes, assisted living facilities, or urgent care clinics. Strong knowledge of physiology, anatomy, medical terminology, and diagnostic evaluation techniques. Hands-on experience with EMR/EHR systems such as Epic, Cerner, eClinicalWorks, or Athenahealth. Competence in performing patient assessments, vital signs measurement, medication administration—including injections—and specimen collection/processing. Ability to work effectively with individuals with disabilities or complex health conditions like dementia or infectious diseases. Basic life support (BLS) certification required; additional certifications such as ACLS or PALS are a plus. Excellent communication skills combined with a compassionate approach to patient care in fast-paced settings like emergency rooms or ICU units.
