Licensed Vocational Nurse (LVN) / Clinical Operations Coordinator Horizon Health | Tyler, TX www.horizonhealthspan.com About Horizon Health Horizon Health is a physician-led longevity and wellness practice dedicated to helping patients optimize healthspan, performance, and quality of life. We combine evidence-based medicine with nutrition, fitness, advanced diagnostics, and lifestyle interventions to create personalized health plans. We are a cash-pay practice, which means no insurance billing and a simpler, more patient-focused workflow than a traditional clinic. We are seeking an energetic, compassionate, and highly organized LVN to become an integral part of our team. This is not a traditional nursing position. The ideal candidate will play a central role in both patient care and daily practice operations, working closely with the physician in a highly personalized healthcare environment. What Makes This Position Different Daytime outpatient setting — no hospital environment No nights, no weekends Close, direct collaboration with the physician Opportunity to learn longevity, preventive medicine, nutrition, and wellness-based care Meaningful, ongoing relationships with patients Small team where your contributions directly impact patient outcomes Clinical Responsibilities Room patients and obtain vital signs Administer medications and treatments as directed Perform phlebotomy and specimen handling Start IVs and assist with IV therapies Assist with diagnostic testing and patient assessments Maintain accurate, timely medical documentation Educate patients on treatment plans, supplements, lifestyle recommendations, and follow-up care Monitor patient progress and communicate concerns to the physician Practice Operations Responsibilities Manage patient scheduling and appointment coordination Answer patient phone calls and electronic communications Collect payments and assist with related processes Coordinate patient follow-up and recall systems Maintain inventory and clinical supplies Support front desk operations as needed Help ensure an exceptional patient experience from first contact through ongoing care Required Qualifications Current Texas LVN license Current BLS certification IV certification and competency (or willingness to obtain promptly) Phlebotomy experience Strong organizational and multitasking abilities Excellent communication and interpersonal skills Proficiency with electronic medical records Preferred / Nice to Have Experience in primary care, internal medicine, wellness, or outpatient practice Familiarity with or interest in nutrition, supplementation, and preventive health Ability to work independently and take initiative Ideal Candidate Dependable, detail-oriented, and professional, with a genuine enjoyment of helping people improve their health. Compensation:
