We are committed to clinical excellence, compassion, and improving patient outcomes. Position Overview We are seeking an experienced Licensed Vocational Nurse (LVN) to join our team on Full TIme. The ideal candidate must have prior Home Health experience and be knowledgeable in providing skilled nursing services in accordance with each patient's care plan.

Responsibilities:

Provide direct patient care in the home setting Perform skilled nursing interventions as ordered by the physician Monitor and document patient condition, progress, and responses Communicate effectively with patients, families, and interdisciplinary team members Ensure compliance with agency policies, procedures, and state/federal regulations

Requirements:

Active Texas LVN License in good standing Home Health experience required Reliable transportation and valid driver's license Strong clinical assessment and documentation skills Ability to work independently and manage time efficiently
