We are seeking a compassionate, organized, and proactive Licensed Vocational Nurse (LVN) to support and help oversee our Collaborative Care Management (CoCM) program. This role works closely with primary care providers, behavioral health care managers, psychiatric consultants, and administrative staff to coordinate patient care, support program workflows, and ensure high-quality patient engagement and follow-up. The ideal candidate is detail-oriented, comfortable working with behavioral health populations, and experienced in care coordination, patient communication, and clinical documentation. Key ResponsibilitiesCare Coordination & Patient Support Assist in coordinating care for patients enrolled in the CoCM program. Conduct patient outreach, follow-up calls, and appointment reminders. Support patient engagement and monitor ongoing participation in treatment plans. Document patient interactions and updates accurately within the EMR. Track patient progress and escalate clinical concerns to the care team when appropriate. Program Oversight & Workflow Support Help maintain daily workflows and operational processes for the CoCM program. Monitor patient registries, tracking systems, and follow-up schedules. Ensure timely completion of required documentation and compliance standards. Assist with quality improvement initiatives and reporting needs. Collaborate with leadership to identify workflow improvements and patient care opportunities. Team Collaboration Work collaboratively with providers, behavioral health specialists, psychiatric consultants, and office staff. Participate in team meetings and case review discussions as needed. Support communication between patients and care team members. Maintain confidentiality and professionalism in all patient interactions. QualificationsRequired Current Licensed Vocational Nurse (LVN) license in the State of Texas. Minimum of 1-2 years of clinical nursing experience. Strong communication and organizational skills. Experience with EMR systems and clinical documentation. Ability to manage multiple tasks and work independently. Compassionate and patient-centered approach to care. Preferred Experience in behavioral health, psychiatry, primary care, or care management. Familiarity with Collaborative Care Management (CoCM) or chronic care management programs. Experience with patient outreach and care coordination. Bilingual (English/Spanish) preferred.
