Position Summary:

The Licensed Practical Nurse (LPN) or Registered Nurse (RN) will provide compassionate and skilled nursing care to residents in the Memory Care unit during the night shift. This role requires a dedicated professional who can deliver high-quality care tailored to the unique needs of veterans with memory impairments, ensuring their safety, comfort, and dignity.

Key Responsibilities:

Provide direct nursing care to residents in the Memory Care unit during the night shift. Monitor and assess residents' physical and mental health status regularly. Administer medications and treatments as prescribed by physicians. Document resident care accurately and timely in accordance with facility policies. Collaborate with interdisciplinary team members to develop and implement individualized care plans. Respond promptly to resident needs, emergencies, and behavioral changes. Ensure a safe and therapeutic environment for residents with memory impairments. Provide support and education to residents and their families as appropriate. Adhere to all regulatory and facility standards related to nursing practice and memory care.

Qualifications:

Current and valid LPN or RN license in the state of Utah. Experience in memory care, geriatrics, or long-term care preferred. Strong clinical assessment and critical thinking skills. Ability to work independently and as part of a multidisciplinary team. Excellent communication and interpersonal skills. CPR certification required or willingness to obtain upon hire. Ability to work night shifts (NOC) as scheduled.

Work Environment:

This position is based at the Southern Utah Veterans Home located in Ivins, St. George, UT. The role involves working primarily during the night shift in a residential care setting dedicated to veterans with memory care needs.
