Job Description

Job Summary Position Summary:
The LPN is a vital part of the clinical team in the 23-hour observation, is responsible for providing medical and psychiatric support to individuals experiencing a behavioral health crisis. The LPN ensures clients' medical needs are assessed and addressed while promoting a safe, stable, and therapeutic environment during their stay.
Key Responsibilities:
Medical & Psychiatric Care:
Monitor and assess clients' physical and mental health status regularly. Administer prescribed medications and monitor for side effects or reactions. Assist in developing, reviewing, and updating individual service and nursing care plans. Document client observations, vital signs, and treatment outcomes per agency and licensing requirements. Maintain proper medication documentation and ensure secure storage and administration practices.
Crisis Support & Safety:
Respond to medical or psychiatric emergencies under the supervision of an RN or provider. Support de-escalation techniques and ensure safety protocols are followed. Collaborate with clinical staff during crisis interventions and stabilization planning.
Coordination & Communication:
Communicate medical concerns and changes in client condition to RNs, physicians, or providers promptly. Participate in daily shift reports and interdisciplinary care team meetings. Coordinate with outside medical providers, pharmacies, and labs as needed.
Compliance & Documentation:
Ensure timely, accurate documentation in accordance with HIPAA, Medicaid, and agency policies. Follow all protocols related to infection control, medication administration, and safety. Assist with audits, reviews, and compliance tasks as needed.
Qualifications:
Education:
Graduate of an accredited LPN program.
Licensure:
Current LPN license in the state of employment (unrestricted). CPR/First Aid certification required (or obtainable within 30 days).
Experience:
At least 1 year of experience in behavioral health, psychiatric, or crisis setting preferred. Familiarity with trauma-informed care and crisis intervention practices is a plus.
Skills:
Strong clinical judgment and attention to detail. Ability to work in a fast-paced, team-oriented environment. Proficient in documentation and electronic health record systems. Compassionate communication and patient-centered approach.

Schedule:
Full-time or part-time; shifts will include evenings, nights, weekends, and holidays.
Compensation:
$25-$30/hr

$25.00 - $31.00 per hour
