The Lodge at Old Trail - Crozet, VA Full‑time | Evening Shift The Lodge at Old Trail is seeking a compassionate and highly organized Evening Shift LPN Supervisor to support our residents and lead our evening care team. This role provides clinical oversight, medication management, family communication, and support to caregivers in both Assisted Living and Memory Care (Seasons). Key Responsibilities Provide clinical leadership and support to evening shift staff Administer medications, injections (including GLP‑1), and treatments Complete dressing changes until home health begins Conduct weekly medication cart audits and bi‑weekly MAR‑to‑cart audits Ensure medications are reordered timely and follow up with pharmacy as needed Monitor temperature logs, supply inventory, and charge capture Oversee Medication Aides in Seasons Respond to emergencies and complete incident reports Notify family and physician of changes in condition Perform pacemaker checks, Dexcom changes, oxygen tubing changes, and nebulizer cleaning oversight Complete monthly vitals and monthly skin assessments (Seasons) Ensure documentation and tasks are completed accurately and on time Participate in shift crossover and walking rounds Assist with onboarding and training of evening staff Support resident engagement by ensuring an evening activity occurs Maintain clean, inviting common areas Communicate updates and concerns to the Director of Health & Wellness Complete Health & Wellness projects as assigned Qualifications Current LPN license (Virginia) Experience in assisted living, long‑term care, or memory care preferred Strong clinical assessment and communication skills Ability to lead and support a team Excellent organization and attention to detail PCC experience a plus Why Join Us?
