Job Description
Overview:
We are seeking a dedicated and detail-oriented Care Coordinator with a Licensed Practical Nurse (LPN) license to join our team. The ideal candidate will be familiar with CMS guidelines, Clinical Review, & QA to review home health referrals for accuracy, completion, and compliance. As a Care Coordinator, you will play a crucial role in ensuring smooth operations while providing vital assistance to field teams.Key Responsibilities and Tasks:
Coordination and Logistics:
Oversee scheduling, resource allocation, and logistics for home health care services. Ensure timely delivery of care and services by coordinating with relevant team members.Support and Problem Solving:
Provide ongoing support to field teams, solving issues as they arise. Address any challenges related to care delivery, ensuring that they are handled promptly and efficiently.Communication and Collaboration:
Maintain open lines of communication with care providers, patients, and families. Collaborate with internal teams to ensure patient care is consistent and meets established standards.Resource Management:
Ensure that necessary resources are available for caregivers and other team members. Oversee inventory and equipment to support seamless service delivery.Monitoring and Evaluation:
Monitor ongoing patient care and services for compliance with CMS guidelines. Review referrals and documentation for accuracy, compliance, and completeness. Evaluate the effectiveness of care provided and suggest improvements as needed.Qualifications:
Required:
Active Licensed Practical Nurse (LPN) license in Virginia. Strong understanding of CMS guidelines and regulations. Previous experience in home health services is required Excellent communication, organizational, and problem-solving skills. Ability to work collaboratively with multidisciplinary teams.Clinical Review:
Ability to perform clinical review of patient documentation, ensuring the quality and accuracy of care provided. Quality Assurance (QA): Experience with QA processes to monitor and improve the quality of care, ensuring compliance with regulatory standards and company policies.Job Type:
