Job Description

Pleasant View Inc is currently accepting applicants for Full-Time LPN for rotating shifts located in Harrisonburg, VA. The LPN will be working at our 8 bed Intermediate Care Facility. The LPN will work under the supervision of the Program Manager. General Knowledge, Skills, and Abilities v Licensed in the state of Virginia to work as a Practical Nurse with the credentials to administer medications. v include but are not limited to:
  • Effective English-written and verbal communication skills.
  • Proficiency in MS Word and Excel
  • Ability to use electronic health records, including MARs
  • Ability to attend and pass required training.
  • Distribute medications and perform treatments to individuals following approved guidelines.
  • Ability to access all programs and areas of the facility and adjacent outside property in a safe and timely manner.
  • Ability to push, pull, bend, reach, walk, stand for extended periods, climb stairs, lift, stoop, and assist an individual to move as needed. Ability to help lift an individual and to use mechanical lifts.
  • Stay awake for the duration of the shift.
  • Ability to consistently come to work and have good attendance.
  • Possession of a current, valid driver's license for three (3) years with an acceptable driving record.
  • Ability to adhere to all safety rules, policies, and guidelines.
  • Ability to become familiar with and uphold the philosophies, goals, and policies of Pleasant View, Inc.
  • Ability to perform direct care.
  • Ability to maintain confidential information
  • Ability to take calls on a rotating weekend basis for medical concerns.
Duties:
v The LPN is hired specifically to ensure that quality nursing and direct care are administered to all individuals who are under their care. Specific duties include, but are not limited to, the following: 1. Plan, implement, and evaluate training programs under the supervision of the Program Managerand follow the guidelines of the planning team. All care, treatment, and training will be designed to promote good physical and mental health and to maintain and/or enhance the consumer's functioning in the community-based residential setting. 2. Accompany individuals on medical appointments, advocating for the individuals to ensure quality medical care. Complete all the necessary documentation to communicate updates to the entire team and to meet regulatory requirements. 3. Ensure the nursing care plan, created by the RN consultant, is carried out as written. 4. Complete quarterly reports and present at the monthly Interdisciplinary Team meeting. 5. Provide direct care as needed, bathing, assisting with personal care needs and transportation. Work a variety of hours to provide nursing support for all shifts (for example: 6am-2pm, 7am-3pm, 8am-4pm,3p-11p). 6. Work effectively with those internal and external to the agency, including interdisciplinary team members, medical providers, direct care staff, and PVI administrative teams. Meet regularly with the planning team, which coordinates the Individual Program Plan. v 1. Maintain accurate and up-to-date medical information as assigned by the Program Manager and according to the policies of Pleasant View, Inc., DBHDS, DMAS, and other regulatory agencies. 2. Document all required information in the medical notes using electronic health records. 3. Maintain accurate records of medical appointments, medications, incident reports, medical PRN notes, and all communications needed on behalf of the individual. 4. Communicate individuals' status to staff members, Team Leads, and Program Manager through staff communication logs. Maintain contact with family members, authorized representatives, and guardians. 5. Complete all required training courses. 6. Assist individuals as needed with medication administration following the statewide guidelines. The LPN provides medication administration training to incoming med-tech trained staff members. 7. Responsible for taking calls from direct care staff, on a rotating weekend basis, for medical concerns that arise. 8. Perform other duties as requested by the program manager, senior program manager, and directors.
