Job Description

LPN/RN Home Health
  • Wednesdays 9am-3pm Leesburg, VA 20177 $37
  • $45 an hour
  • Part-time, Full-time $37
  • $45 an hour
  • Part-time, Full-time 1st Americare is seeking an LPN/RN to work with our client in the Leesburg area.
Candidate with a G-tube experience is preferred. Spanish speaking candidate preferred. The schedule for this case will be: Wednesdays 9am-3pm
Duties and Responsibilities:
  • Provide skilled nursing care to patients in their homes
  • Conduct comprehensive assessments of patients' health status
  • Develop and implement individualized care plans
  • Administer medications and treatments as prescribed by physicians
  • Monitor and evaluate patients' progress and adjust care plans accordingly
  • Educate patients and their families on managing their conditions and promoting wellness
  • Collaborate with other healthcare professionals to coordinate patient care
  • Document patient care activities accurately and timely
